This facet brings greater focus, resources and coordination for health initiatives to address disparate chronic health issues in the community.

Mission/Purpose: To promote and facilitate programs that support the maintenance of good health and the elimination of chronic health disparities in communities of color through education, health advocacy, and optimal utilization of health resources.

Areas of Emphasis: Cardiovascular disease, breast cancer, and organ, tissue and blood donation
National Initiative: Childhood Obesity
Signature Programs: HeartLinks to Heart Health, Linkages to Life: Organ, Tissue and Bone Marrow Donation Program, and Walk for Healthy Living
